BASIC PURPOSE: Will function both as a nursing assistant and unit secretary.
Assists with the care and flow of patients on the Acute Care Unit.
ESSENTIAL FUNCTIONS FOR CNA Major Job Functions: Assisting with patient care and hygiene.
Monitoring the on-going needs of patients.
Functions as a unit secretary, answering the phone, managing the flow of charts, filling out requisitions.
Assists during the medical and nursing care of patients.

Transports patients: wheelchair, cart, in and out of vehicles.
Monitors vital signs.
Prepares patients for meals and feeds those who require assistance.
Answers call lights promptly.
May apply dressings that do not require medications.
Obtains specimens and logs them into the lab as required with proper labeling.
Stocks supplies as needed.
Reports changes in patient condition or pertinent observations to charge nurse.
Follows confidentiality policies regarding patient information, and respects patient confidentiality.
Meets the MHHS Nursing Standard of Care.
Other duties as assigned.
SAFETY: Consistently uses good body mechanics.
Complies with established emergency and infection control procedures.
Attends mandatory meetings.
Consistently uses universal precautions.
Follows all facility and departmental policies and procedures in performance of all job duties, to ensure a safe work environment for self and others.
TIME MANAGEMENT: Prioritizes tasks regarding assignments.
Will staff as scheduled or cover own shift through means of trading.
Will comply with vacation policy.
In the event of a call off, two hours notice of such will be given MHHS by the CNA calling off.
Will comply with scheduling policy, may have change in shifts, weekends and holidays.
Will follow all hospital/departmental policies.
Will respond to disasters if possible.
Completes work in a timely manner, takes time to be helpful with others.
Requires general/minimal supervision.
Responds to change in the work environment in a way that promotes the facilities goals and objectives.
PHYSICAL REQUIREMENTS: Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shifts.
Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
Must be able to read and write legibly in English.
Hearing within normal range is required (i.
e.
, to assess breath sounds, bowel sounds, apical pulse, monitors, etc.
) Visual acuity must be within normal range.
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
MENTAL REQUIREMENTS: Must have the ability to work successfully under highly stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.
Must have the ability to make sound, independent judgments based on scientific principles, and also be able to collaborate with other multi-disciplinary team members in an appropriate fashion.
Must have the ability to comprehend and perform oral and written instructions and procedures.
Must have effective reading and comprehension skills.
Must have strong communication skills, written and verbal.
ENVIRONMENTAL CONDITIONS (POTENTIAL FOR): Exposure to body fluids, human tissues, contagious diseases, sharps, and explosive gases.
Exposure to cleaning agents and disinfectants.
Exposure to toxic gas, fumes, and odors.
Exposure to high stress and constant interruptions.
Exposure to electrical current.
Exposure to radiation from x-rays.
JOB DUTIES ELECTRONIC HEALTH RECORDS (EMR) Demonstrates capabilities of use of computers and EMR, documenting required data, vital signs, call back and electronic forms.
VITAL SIGNS AND PATIENT MONITORING Telemetry hook-up and 6 sec.
recording Correctly measures and records vital signs Measures and records intake and output Records meal % taken ADMISSIONS Works with care team in order to facilitate admission standard Prepares room for admission DISCHARGE Works with care team to facilitate discharge: Assists patient with getting dressed and gathering belongings.
Transport patient in wheelchair to vehicle TRANSFER Works with care team in order to facilitate transfer in compliance with transfer standard Assists the patient/family in preparing patient for transfer.
ASSESSMENT/OBSERVATION Reports observed changes in patient condition to charge nurse.
Reports abnormalities in vital signs to charge nurse Assists with all aspects of patient care, using observatory skills.
Documents fully and accurately in EMR PATIENT TEACHING Reviews with patient safety measures with every contact BASIC PATIENT CARE Provides a clean, safe, calm, quiet, and comfortable environment per the basic care standard.
Ensures privacy and demonstrates concern for dignity of patient in performing all patient care.
Completes assignments in a timely manner.
Works as a team member according to the standard.
COMMUNICATION Serves as patient advocate, acting as liaison between patient, family, and the physician/RN to provide continuity of patient care.
Relays critical information to the appropriate staff regarding patient condition.
Demonstrates knowledge of and adherence to patient rights.
Maintains patient confidentiality according to policy.
Uses equipment appropriately in order to maintain flow of information.
EQUIPMENT Demonstrates the ability to use the following equipment efficiently, safely, and appropriately.
Automatic vital signs machine with pulse oximeter Manual BP cuff and thermometer Personal Protective Equipment (PPE) Telemetry equipment Bed and chair alarms Bed scales Lab specimens SUPPLIES Charges all chargeable supplies for patient.
Maintains adequate supplies in various areas to maintain flow of work with minimal interruption.
QUALITY IMPROVEMENT (QI) Participates in QI activities to meet quality measures utilizing Evidence Based Guidelines established in the EMR.
Develops an action plan (PDSA) in areas identified needing improvement and works with Care Team to achieve goals for quality improvement.
Shares data in monthly clinic staff & IQM meetings.
SAFETY Demonstrates knowledge of and compliance with safety policies and procedures QUALIFICATIONS: Required to be certified, or eligible to be certified by the State of West Virginia as a Nursing Assistant.
